The Emergency Room Butterfly Effect Tool


In a bustling Emergency Room, the analysis revealed a surprising insight: the Door-to-Provider time for Discharged Patients shows the highest correlation with the ER's Peak Census, surpassing bed holding and transfers. While initially overshadowed, addressing this aspect can significantly enhance the experiences of both inpatient and outpatient ER admissions. This unforeseen revelation underscores the power of robust analytics in streamlining processes and fostering agreement on implementing pilot changes.

A case study in an Emergency Department grappling with bed holding highlights the transformative impact of redefining the core issue. Instead of solely focusing on Length of Stay, a new metric, Peak Census, was introduced to predict and mitigate peak influxes effectively. A predictive regression model was developed so that the ER and Hospital could reliably predict ER evening peak census based on a number of metrics measured at noon. This was used to determine at what peak census overnight stays would happen. When this innovative "Butterfly Effect Model" predicted overnight stays at noon, this triggered a new protocol of ER, OR, IP and Hospital management activities around ER and PACU discharges, Critical Care Transfers, Med Surg Discharges and both ER and Inpatient nurse and provider staffing for the PM shift. The result was a dramatic reduction in overnight stays in the ER while also making modest improvements in inpatient and ER Length of Stay, and reductions in bed holding. Surprisingly, the actual volume of insured patients in the ER grew as well, as word about improved throughput and attentive care got out, improving the bottom line.


This emphasis on prediction and early, engineered intervention among a multi-disciplinary team, particularly in optimizing Discharges and Transfers, to preemptively manage surges in the ER census proved effective. But it had to start with actual study in that ER. By strategically implementing changes in inpatient flow and ER throughput, the Butterfly Effect model showcases how minor adjustments early can yield significant reductions in evening peak census, illustrating the ripple effect of proactive measures on operational efficiency.
